Offering: first -time vaccine shots, first and second booster shots, Pfizer and Moderna.

Moderna: available for 18 + years old. Pfizer: available for 12 + years old.

Bring: proof of prior Covid vaccines, photo ID, Insurance card.

You may print and fill out the Covid vaccine consent form prior to coming into the clinic.

No appointment needed, just call to let us know you are coming! 

There is no charge to you.
